All applications for a permit to construct driveways, streets, sidewalks, curbs, gutters,
pipelines, to widen the paved roadway, to cut pavement, sidewalks, curbs, gutters
or to repave pavement cuts, shall be in writing on forms provided by the city, and
shall be accompanied by at least three (3) copies of a plot plan showing the size
of the property, the location with reference to lot and street lines of existing proposed
buildings, driveways, curbs, sidewalks, trees, poles or other objects that might affect
the work.
